The Bloodied Skull was a consumable item dropped by Skeletron when it spawned in Eternity Mode if Skeletron has not already been defeated. When used, Bloodied Skull spawned an Old Man and set the time of day to 7:30PM. If an Old Man or Skeletron was already alive, the Bloodied Skull couldn't be used.

If used after Skeletron is defeated, Bloodied Skull would instead spawn a Dungeon Guardian.
