头骨挂坠是一个困难模式之前的永恒模式独有饰品。装备后，它会提供对眩晕减益的免疫，以及15%的伤害加成，但同时减少10%的伤害减免，并降低玩家400点仇恨。它还会在玩家头上召唤一个水晶头骨仆从，并在玩家按住攻击键时充能。充能完毕时（发出紫色粒子），松开攻击键会快速发射暗灵弹。暗灵弹的基础伤害为30。
